Uma história de usuário completa contém três elementos.
- persona ( quem? ): quem quer usar isso
- ação ( o quê? ): que ação concluir
- Valor: ( por que? ) faça isso e que valor pode ser trazido fazendo isso

As informações descritivas das histórias de usuários são escritas em cartões de papel de forma manuscrita tradicional, então Ron Jeffries (2001) chama esses três aspectos de 3C:

Cartão: as histórias do usuário geralmente são escritas em pequenos cartões com uma breve descrição da história, regras e critérios de conclusão.
